   Example could be as followes:
   -felt some discomfort in shoulder so lightened weight
   -using cable column at condo gym (weights dont match exactly)
   -used black band for last 2 chinups to complete the set

   Being able to view them when looking at history for the exercise or looking at progress page it could be a note icon that when clicked it expands the note

6. Allow notes to be written for each individual set rather than only the overall workout. This provides much better feedback for the trainer
