Customizable individual client check in forms
Is there a way to customize client check in forms , i have many clients. Eg client A is great with macros, client B isnt good with macros. I don't want to send a generic check in form to all clients about macros. IS there a way to make each check in unique?
Hi!
You voted for customizable individual check-in forms. The ability to create custom Forms is now available in ABC Trainerize!
We’re continuing to build out tools shaped by feedback from the community.
What’s included:
- Create fully customizable forms from the Master Libraries, including daily wellness surveys, weekly check-ins, and program feedback
- Build tailored forms for different client groups (e.g., Nutrition vs. Wellness), or customize forms for individual clients as needed
- Add a variety of question types, such as multiple choice, single select, text responses, or 1–10 scales
- Review submissions side by side within a client’s profile to track progress and identify trends over time
Forms are designed to give you flexible control over what you track and how responses are collected, so you can tailor them to you, and your client's needs.

It would be great to personalize check-in forms for each client individually like I can with google forms.

YES THIS!!! I have personal training clients, habit clients, nutrition clients, and clients I do sleep/stress groups with... There needs to be more than 1 check in form otherwise having this check in option is pointless.

Agree with the above comments - I have clients who see me for just training, so I'd like to create a personal one for them. This will look more personal for my clients.

Would love to have access to multiple forms, as well! My first is a workout readiness check-in for my in-person clients who have specific programming needs, and the second would be an end-of-week review to see how everyone is feeling to get an idea of if we need to ramp up or down the following week.
