    I feel this should be a pop up or something so as soon as the client opens the app, the client HAS TO FILL OUT a customizable daily form to access the normal app.

    I think it would be easiest to simply have this pop up form use the same coding approach that the built in consultation form has -- That way, we the trainers can simply customize the questions and ratings as we choose.

    If the data can be "added" up for weekly averages on a clients dashboard, that would be ideal!!
